Accident process
On January 25, in the steelmaking plant of a company, during the production preparation of the new converter, Mr. Zhu stood at the bottom of the small room on the gap between the front platform and the converter mouth, and threw wood into the converter. Accident causes and lessons
Direct cause:
The operators fell to the slag pit and died.
Indirect cause:
1. Operators have weak safety awareness and fail to comply with the provisions of "pay attention to safety, wear safety belts and strengthen protection" in the general safety regulations of the post.
2. The on-site safety disclosure was not rigorous, and the safety condition of the operation site was not carefully checked before the operation process.
3. For the operation mode of using the small room to deliver firewood before drying, there is a lack of awareness of the hidden dangers of production safety accidents, and the hidden dangers are not carefully investigated.
Lessons learned
1. Strengthen the education, training and inspection of employees, improve the regulations, and urge employees to strictly implement them.
2. The management personnel shall carry out targeted safety disclosure before operation and strengthen the safety supervision of non-regular operations.
3. Optimize the operation mode of converter drying, strengthen the identification and control of hazard sources in the workplace and the whole process of production process, and implement the safety production responsibilities at all levels.
